This is the clinical case of a middle-aged patient, who is admitted for severe acute pancreatitis with an unfavorable evolution, who with the respective support and management emerges from the process. When he presents persistent febrile peaks, we try to rule out new concomitant infections, he is covered with antibiotics and requests complementary tests such as images, contrast-enhanced tomography, when the patient presents exanthematous lesions on the entire body surface with marked elevation of eosinophils, the challenges of this case were several, first, defining what type of allergic reaction was added, what was the cause of said allergic reaction whether antibiotics, contrast or other drugs and most importantly, what was the etiology of the distributive, septic or anaphylactic shock, the dilemma concerned more about the therapeutic conduct on the corticosteroid pulse among others.
