Abstract

Virilizing adrenocortical adenoma is a rare neoplasm characterized by excessive androgen production. Its incidence is low, and it is more frequent in adult women. It presents with signs of rapid-onset hyperandrogenism. Diagnosis is based on clinical suspicion and biochemical and imaging confirmation. The treatment of choice is surgery. We present the case of a 43-year-old woman with signs of hyperandrogenism and elevated total testosterone and DHEAS levels. Adrenal CT scan: solid, rounded mass with internal calcifications. She underwent a left adrenalectomy; pathology showed adrenocortical adenoma without evidence of vascular invasion, and immunohistochemistry: inhibin positive and KI67 of 2%. Postoperative hormonal monitoring was normal. Despite its rarity, timely recognition is essential, given that its differential diagnosis includes malignant entities with a worse prognosis. Follow-up is recommended to confirm complete resolution and rule out recurrence.
